In addition to the paper forms we provided you and to provide us with as much information as possible about your child and your concerns before we meet, we ask that you complete a set of confidential questionnaires in CHADIS (Child Health and Development Interactive System), an online computer system we use. CHADIS allows you to answer a series of questions over the internet BEFORE your visit. It will ask about your concerns about your child's behavior, development and health. The CHADIS website operates with the highest level of security and the information you provide is strictly confidential. Only our clinical staff can see it.

Completing this information may take an hour or two, depending on the age of your child and the questionnaires your clinician wishes you to complete. If you have not used CHADIS before, follow the directions below. Your cooperation will allow us to truly understand your concerns and help us to serve you well.

How to Complete CHADIS Questionnaires On-Line

The following are step-by-step instructions for accessing and using CHADIS:

First-time User Instructions

1. Click on the "ENTER CHADIS HERE" link above.

2. Complete the "New Users" section, entering your first and last name and email address. Your complete email address will be your logon in the future. You should also select a password that only you will know. (Make sure you keep a copy of your password as you will need it to access CHADIS in the future.)

3. Select the clinician who is working with you and your child from the "Choose a Doctor" menu.

4. When you have finished, add this page to your "Favorites" on your web browser and then click the "First Visit to CHADIS" in the "appointment types" drop down menu. You will then be directed to the questionnaires we wish you to complete.

5. To complete the questionnaires, click on "Begin" next to each assigned questionnaire to answer the questions. You may be asked if you will see your clinician within two weeks. Please ignore this prompt and complete the questionnaires regardless of the length of time before your next appointment.

6. To answer questions, click on the "bubble" next to the answer you want to give for each question and then click "Continue" at the bottom of the page until you have finished.

6. When you have completed a questionnaire and are satisfied with your answers, click "Submit Now."

For all questionnaires EXCEPT THE DSM-PC, you may stop at any point and go back later to finish it by clicking "Save & Quit at the end of the page. For the DSM-PC, you must start from the beginning and complete the questionnaire in one session, so complete this questionnaire when you are sure you have at least an hour.

You may retake a questionnaire (except for the DSM-PC) from the beginning if you change your mind about your answers as long as you have not yet clicked on the "Submit Now" button.

Returning Users

1. If you have placed the CHADIS Welcome Page in your favorites, you may go there directly. Otherwise, you may click on the "CHADIS Sign In" link above.

2. On the "Returning Users" section of the CHADIS Welcome Page, enter your email address under "Username" and the password you selected to enter CHADIS.

To complete additional questionnaires:

When asked to complete a second set of questionnaires before your next appointment, follow these simple steps:

1. Select your clinician from the "Choose a Doctor" drop down menu.

2. Select the "appointment type" from the drop down menu. For example, if you have completed the first set of questionnaires for your first visit and now will be returning to see your developmental/behavioral pediatrician, please select "Developmental/Behaviorial Pediatrics" to complete your next set of questionnaires.

If your clinician has asked you to complete other questionnaires, they will instruct you to select "Additional Questionnaires Requested by my Team" in the "appointment type" drop down menu.

3. Follow the steps for completing each questionnaire.

To add another child/sibling:

CHADIS will establish a single record for each child in your family. To add a child complete the following steps:

1. Open the record for the first child you entered in CHADIS.

2. Click on "Add Another Child" on the lower section of this page.

3. Enter the new child's information as prompted.

4. Select your clinician from the "Choose a Doctor" memu.

5. Select "First Visit to CHADIS" in the "Appointment Types" drop down menu and complete the questionnaires as previsously described.
